This form is required to request contract rent increases for units subsidized with a MRVP voucher or for new units submitted for lease-up with MRVP. Rent increases must be submitted at least 60 days ahead of the lease renewal date, with either the tenant’s signature on the form, or written notice to the tenant attached.
